GM1 gangliosidosis type 2

GM1 gangliosidosis type 2 is a lysosomal storage disorder that affects multiple organ systems, most the brain, skeleton, and liver. It results from pathogenic variants in the GLB1 gene, which lead to deficient beta-galactosidase activity and accumulation of GM1 ganglioside in cells. Symptoms usually appear after a period of apparently normal early development, with onset of problems between about seven months and three years of age. After a period of apparently typical early development, affected children experience a phase of developmental regression marked by loss of previously acquired motor and cognitive skills. When the enzyme is deficient, these substances accumulate, damaging cells throughout the body, especially neurons and bone-forming tissues. The disorder is inherited in an autosomal recessive pattern. This means an individual must receive one altered GLB1 gene from each parent to develop the condition. Parents who each carry one pathogenic variant (carriers) are typically healthy but have a 25% chance with each pregnancy of having an affected child. This condition is more common when parents share a common ancestor, because relatives are more likely to carry the same rare genetic variant. Nutritional support, sometimes including gastrostomy feeding, can help mitigate failure to thrive. Orthopedic interventions may be needed for progressive scoliosis or kyphosis. Long-term management includes regular surveillance for potential complications. Regular surveillance is an essential part of long-term care, helping to identify potential complications before they become serious. Suggested monitoring includes periodic liver imaging, cardiac evaluation to follow patent ductus arteriosus or other anomalies, assessment of spine curvature, and developmental assessments to guide therapy needs. No FDA-approved disease-modifying treatments are currently available, but several investigational approaches—including enzyme replacement, substrate reduction, and gene therapy—are being studied in clinical trials.
