Blistering typically begins in childhood or adolescence and is usually limited to the shins and feet, although other areas can be involved. The hallmark of the disorder is recurrent blistering and erosions on the pretibial (shin) areas after minor friction or trauma. In many families the condition follows an autosomal dominant pattern, meaning one altered copy of COL7A1 is sufficient to cause disease and each child of an affected parent has a 50 percent chance of inheriting the variant. In other families it is autosomal recessive, in which two altered copies are required; parents who each carry one variant are typically unaffected carriers and have a 25 percent chance with each pregnancy of having an affected child. Because the condition is extremely rare, it may be more common when parents share a common ancestor, as the chance that both carry the same rare variant is higher. Genetic counseling is recommended for affected individuals and their relatives to clarify personal risks, carrier testing options, and family-planning choices.

Currently, management focuses on supportive care and symptom control. Gentle wound care using non-adhesive dressings, topical antiseptics to reduce infection risk, and moisturizers to minimize friction are mainstays of treatment. Oral antihistamines or topical agents may help relieve bothersome itching.
