Mucopolysaccharidosis type 3A (often shortened to MPS IIIA or Sanfilippo syndrome type A) is a lysosomal storage disorder that mainly affects the brain and, to a lesser extent, other body systems. It occurs when changes in the SGSH gene reduce or eliminate the activity of heparan sulfate sulfatase, an enzyme needed to break down a complex sugar called heparan sulfate. Without adequate enzyme activity, undegraded material builds up inside cells, especially in the central nervous system, leading to progressive neurological symptoms. The condition usually becomes apparent in early childhood and affects boys and girls equally. It is considered rare, affecting roughly one to nine individuals in every million people.

Severity and symptoms vary widely between individuals, and not all experience every feature. The condition is more common when parents share a common ancestor, because they are more likely to carry the same rare genetic change. Carrier testing and genetic counseling can clarify risks for future pregnancies and identify other relatives who may also be carriers.

Currently, management focuses on supportive care and symptom management. Treatment is tailored to each individual’s needs and typically involves a multidisciplinary team that may include metabolic specialists, neurologists, gastroenterologists, ear-nose-throat physicians, and rehabilitation therapists. Regular surveillance is an essential part of long-term care, helping to identify potential complications before they become serious. Interventions may address sleep disruption, control seizures, support communication and learning, manage gastrointestinal issues, and treat hearing loss. Families are encouraged to work closely with their care team to develop individualized plans and to revisit these plans as needs change over time.
