Fragile X syndrome is an inherited neurodevelopmental condition that primarily affects the brain and, to a lesser extent, connective and facial tissues. It is caused by a CGG repeat expansion in the FMR1 gene, which reduces or silences production of the fragile X mental retardation protein that supports healthy synapse development. Because the gene is located on the X chromosome, males are usually more severely affected than females. The condition is relatively common among genetic disorders, occurring in about 1 in 4,000 boys and 1 in 8,000 girls, and carriers of the premutation are even more frequent in the general population.

Moderate intellectual disability is consistently observed in affected males, while females often have mild learning difficulties. Characteristic facial features include a long face, prominent ears (macrotia), and a pronounced jaw, features that tend to become more apparent with age. Many boys develop enlarged testes after puberty. Early developmental signs often include speech delay, reduced eye contact, hyperactivity, and repetitive movements such as hand flapping. As children grow, anxiety, attention-deficit traits, and behaviors associated with the autism spectrum can emerge, and some individuals engage in self-biting during periods of stress. Less common findings include joint hyperextensibility, a broad forehead, and flexible finger joints. Not all individuals experience every feature, and severity varies widely even within the same family.

Fragile X syndrome results from an abnormal expansion of CGG repeats within the FMR1 gene. A normal allele carries up to 44 repeats, a premutation carries 55–200 repeats, and a full mutation exceeds 200 repeats and usually becomes methylated, preventing the gene from producing functional protein. The disorder follows an X-linked inheritance pattern: a male who inherits a full mutation from his mother will be affected, while a female with one full-mutation allele may have milder symptoms because her second, healthy X chromosome can partly compensate. Repeat length can change when passed to the next generation, so premutation carriers are at risk of having children with the full mutation. Female premutation carriers can develop fragile X-associated primary ovarian insufficiency, and older male carriers may develop fragile X-associated tremor/ataxia syndrome. Carrier testing and genetic counseling are recommended for at-risk relatives, including women who wish to plan pregnancies.

Currently, management centers on supportive care tailored to each person’s developmental and behavioral profile. Foundational interventions include early-childhood special education, speech and language therapy to promote communication, occupational therapy to address sensory processing and fine-motor skills, and behavior therapy to help manage anxiety, attention difficulties, and repetitive behaviors. Medications such as stimulants, selective serotonin reuptake inhibitors, or atypical antipsychotics may be prescribed to reduce hyperactivity, anxiety, or severe behavioral outbursts when needed. Care is typically coordinated by a multidisciplinary team that may involve a geneticist, developmental pediatrician, neurologist, psychiatrist, and speech-language pathologist. Regular monitoring of educational progress, mental health, and pubertal changes helps the care team adjust therapies over time. At present, no FDA-approved treatments specifically target the underlying genetic defect, but research programs are exploring drugs that modulate synaptic signaling or attempt to reactivate the silenced gene. Families are encouraged to discuss evolving options with their healthcare providers and to seek genetic counseling for family-planning decisions.

Fragile X syndrome is a lifelong condition, yet outcomes vary widely. Many individuals make steady developmental gains, and with early intervention some achieve functional communication, reading ability, and partial independence in adulthood. Behavioral challenges such as anxiety or social avoidance often persist but can be mitigated through therapy and supportive environments. Females, on average, have milder cognitive involvement and a higher chance of living independently. Quality of life continues to improve as therapies evolve and educational supports become more widely available, but ongoing assistance is typically needed to navigate academic, occupational, and social settings throughout life.
