Beck-Fahrner syndrome

Beck-Fahrner syndrome is a multisystem neurodevelopmental disorder that primarily affects early developmental milestones such as motor and speech skills. It is associated with pathogenic variants in the TET3 gene, which plays a role in epigenetic regulation. Affected individuals typically present during infancy with global developmental delay and challenges in cognitive and social responsiveness. Being extremely rare, the condition affects fewer than 1 in 1,000,000 people.

Clinically, Beck-Fahrner syndrome is characterized by a range of neurodevelopmental and behavioral features. All affected individuals exhibit delayed gross motor development, global developmental delay, delayed speech and language development, and reduced social responsiveness. Many also experience intellectual disability and autistic behavior, with additional features such as delayed fine motor development, anxiety, and seizure activity observed in a significant subset. Other variable findings include traits like a long face, EEG abnormalities, feeding difficulties in infancy, attention deficit hyperactivity disorder, high palate, and various facial features as well as, less commonly, cardiac concerns such as ventricular septal defect. Not all individuals experience every feature, and severity varies considerably.

Beck-Fahrner syndrome is caused by pathogenic variants in the TET3 gene, which is essential for proper epigenetic regulation during development. Multiple inheritance patterns have been reported for this condition, with both autosomal recessive and autosomal dominant patterns observed. In cases where the autosomal recessive pattern applies, the condition tends to be more common in families where parents share a common ancestor.
