Pitt-Hopkins syndrome

Pitt-Hopkins syndrome is a neurodevelopmental condition that primarily affects the brain, face, and autonomic nervous system. Distinctive facial features provide important diagnostic clues. These include a broad mouth with widely spaced teeth, full cheeks, a deep or unusually shaped philtrum, thickened ear helices, a narrow forehead, and sparse inner eyebrows. The eyes may appear deeply set, and the nostrils are frequently flared. Growth and musculoskeletal findings evolve over time. Secondary microcephaly (a head size that becomes relatively small for age) develops in most children. Low muscle tone is common and can contribute to delayed sitting, standing, and walking. Pes planus or pes valgus (flat or outward-turned feet) and tapered or broad fingertips are frequently seen. Single transverse palmar creases may be present. Gastrointestinal issues such as gastroesophageal reflux and chronic constipation occur in many individuals and can affect nutrition and comfort. Episodes of autonomic instability, including changes in breathing pattern and occasional temperature dysregulation, may complicate care. Not all individuals experience all features, and severity varies considerably from person to person.

Pitt-Hopkins syndrome is caused by pathogenic (disease-causing) variants in the TCF4 gene. TCF4 provides instructions for making a transcription factor that helps control the activity of many other genes, especially during brain and nerve development. The condition follows an autosomal dominant inheritance pattern, meaning a change in just one copy of TCF4 is sufficient to cause symptoms. An affected individual has a 50 percent chance of passing the altered gene to each child, regardless of sex. Regular surveillance for growth, nutrition, orthopedic alignment of the feet and spine, and gastrointestinal function helps detect complications early and guide timely interventions. Genetic counseling is recommended for the individual and family members to discuss inheritance and reproductive options.

Pitt-Hopkins syndrome is a lifelong condition. Most individuals achieve some motor milestones, such as sitting and walking, although these may occur later than expected and speech is often limited. Cognitive, behavioral, and autonomic challenges typically persist into adulthood, and ongoing support is usually required for daily living. Outcomes vary depending on the severity of breathing disturbances, mobility, and associated medical issues, as well as access to specialized therapies and educational services.
